Apply for a US Passport
The Canton Public Library is an authorized Passport Acceptance Facility.
We process Passport Applications by appointment only
Choose "SCHEDULE AN APPOINTMENT TO SUBMIT A PASSPORT APPLICATION" and select an available time
Visit the Department of State's website or call 877-487-2778 for questions about Urgent Travel or Life and Death Emergency Travel
The Library Passport Line is 339-502-5721. Agents will return calls as their schedules allow
Passport appointment hours:
- Mondays 2-4pm & 6-8pm
- Tuesdays 2-4pm & 6-8pm
- Wednesdays 2-4pm
- Thursdays 2-4pm
- Friday 2-4pm
- Saturdays* 11am-12pm & 2-4pm
*Saturdays hours limited in the summer
Documentation:
Requirements for children under the age of 16:
- DS-11 form filled out in black ink, SS# is required (Using the Online Form Filler ahead of time at travel.state.gov will save time)
- The child's original birth certificate must be submitted with the application
- If the applicant has a valid U.S. Passport, that Passport must be submitted with the application
- Both parents must appear
- Both parents must present proof of identity
- Both parents must sign the child's application
- Passport photo (We can take pictures here for $10)
Requirements for Young Adults aged 16 or 17:
- DS-11 form filled out in black ink, SS# is required (Using the Online Form Filler ahead of time at travel.state.gov will save time)
- The applicant's original birth certificate must be submitted with the application
- If the applicant has a valid U.S. Passport, that Passport must be submitted with the application
- If the child has no photo identification of their own, a parent may present their photo identification and co-sign the application
- Passport photo (We can take pictures here for $10)
Documentation Requirements for adults over 18:
- DS-11 form filled out in black ink, SS# is required (Using the Online Form Filler ahead of time at travel.state.gov will save time)
- Proof of Citizenship (Original Birth Certificate, Certificate of Naturalization/Citizenship, Passport)
- If the applicant has a valid U.S. Passport, that Passport must be submitted with the application
- Proof of Identity (Valid Massachusetts Driver's License or State ID)
- Passport photo (We can take pictures here for $10)
Fees:
Mandatory Fees payable to the Department of State: (Check or Money Order only)
Passport Fee:
- Adults: $130.00 book and/or $30.00 card, each valid for 10 years
- Children: $100 book and/or $15.00 card, each valid for 5 years
Optional Fees:
- Expedite Fee: an additional $60.00 per application
- Overnight Return Shipping Fee $18.32 per passport
Mandatory Fees payable to the Canton Public Library: (Check, Money Order or Cash)
- Execution Fee: $35/Passport Application
Optional Fees:
- Photo Fee: $10/Photo
- Overnight Shipping Fee: $27.90
Processing Times:
Applicants are encouraged to visit the Department of State's website or call 1-877-487-2778 for the most up-to-date information, as requirements, fees and processing times are subject to change without notice.
Routine Delivery Service (as of November 2022):
- 6-9 weeks from application
Expedited Delivery Service (as of November 2022):
- 3-5 weeks from application
Let us know if you are traveling in less than 9 weeks so that we can inform you of options to expedite your application.
If you are traveling internationally in less than 4 weeks or need to obtain a foreign visa within 4 weeks for a Life or Death Emergency, or for Urgent Travel, you must call 1-877-487-2778 within 2 weeks of your departure and arrange an appointment.
